NAD+ and Healthy Aging: What the Latest Research Shows

NAD+ occupies an unusual position: genuinely central to cellular metabolism, genuinely declining with age, and genuinely oversold by a large number of products.
All three of those can be true simultaneously, which is what makes the category confusing.
What NAD+ does
Nicotinamide adenine dinucleotide is a coenzyme present in every cell. It is required for the reactions that convert food into usable energy, and it is consumed by enzyme families involved in DNA repair and cellular signaling.
When NAD+ availability falls, both processes are affected. That mechanism is why the molecule attracted longevity research attention in the first place.
The decline is real
Tissue NAD+ levels decrease with age across multiple tissue types. This is one of the more consistently reproduced observations in the field.
What supplementation has shown
The commonly studied precursors are nicotinamide riboside (NR) and nicotinamide mononucleotide (NMN). Human trials support a clear first-order finding and much weaker second-order ones.
Established: both precursors raise blood NAD+ levels in humans, dose-dependently, with a reasonable short-term safety profile in the doses studied.
Not established: that raising blood NAD+ produces the downstream clinical outcomes people are buying these products for. Trials measuring physical function, insulin sensitivity, and related endpoints have produced mixed and largely modest results.
Open questions
- Does blood NAD+ reflect tissue NAD+? The tissues of interest are not the ones being measured.
- Which outcomes should be measured? Long-term aging endpoints are slow and expensive to study.
- Who benefits, if anyone? Trials have often used small, relatively healthy cohorts.
- What about long-term safety? Short-term data look reassuring; multi-year data are thin.
- Are women adequately represented? In several trials, not especially.
Where this leaves it
NAD+ is a legitimate area of research and one of the more interesting threads in aging biology. It is also a category where the products have run substantially ahead of the outcome data.
